Adding a Baby to Uzbek Passport:

Uzbek mother who wishes to add her baby to her passport should first get a birth certificate from Uzbekistan in cooperation with the Embassy in Cairo before proceeding in the addition.

Requirements:

1. Handwritten petition submitted by the mother in Uzbek language addressed to the Consul in Cairo
    asking for the addition.

2. Handwritten statement submitted by the spouse showing no objection for the addition.

3. Jordanian Birth certificate legalized from the Ministry of Foreign Affairs.

4. Passport copies of the parents where the mother’s passport copy should include the following 
    pages:

     - Personal Information & Photo
     - Address in Uzbekistan
     - OVIR (Office of Visa & Registration) stamp
     - Visa for Jordan

5. Marriage certificate legalized from the Ministry of Foreign Affairs.

6. Job certificate for the husband legalized from the Ministry of Foreign Affairs.  In case of a student,
    Student Identification Document (شهادة إثبات طالب)  to be provided.

7. Two photos for the mother and three photos for the baby.
